Dextromethorphan HBr, Guaifenesin falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti may increase the levels and clinical effects of drugs metabolized by CYP2D6.
Animal research suggests that fo-ti might inhibit CYP2D6. Additionally, an in vitro study suggests that the degree of CYP2D6 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this interaction has not been reported in humans.

Here's the gist for this cough and congestion combo. The dextromethorphan part of this product is cleared by a liver enzyme called CYP2D6. Lab and animal studies hint that Fo-ti, especially the raw root, may slow that enzyme down. If that happened in people, dextromethorphan could build up a bit, which might mean more drowsiness, dizziness, or in larger amounts a spacey or restless feeling. The guaifenesin piece isn't really affected by this enzyme.

I want to be honest about the evidence though. This is only a theoretical concern from test-tube and animal work, and it has not been reported in actual patients. For most folks taking a short course of cough medicine, this is unlikely to cause trouble. Still, run it by your pharmacist if you use Fo-ti regularly.

In vitro research suggests that fo-ti might inhibit CYP3A4. One in vitro study suggests that the degree of CYP3A4 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this evidence conflicts with animal research suggesting that fo-ti does not inhibit CYP3A4. This interaction has not been reported in humans.

Here's the basic idea: your cough medicine contains dextromethorphan, which your liver partly clears using an enzyme called CYP3A4. In test-tube studies, Fo-ti appeared to slow that enzyme down a bit. If that were to happen in your body, dextromethorphan could linger a little longer and feel stronger, which might mean more drowsiness, dizziness, or a foggy, lightheaded feeling. The guaifenesin part isn't really affected here.

I want to be honest about how soft this evidence is. It comes only from lab dishes, animal studies actually disagreed, and nothing like this has been reported in real people. So I wouldn't lose sleep over it, but if you take Fo-ti regularly, just mention it to your pharmacist before reaching for this cough syrup.
