Why this applies to Pirfenidone

Pirfenidone falls under CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti might increase or decrease the levels and clinical effects of drugs metabolized by CYP1A2.
In vitro research suggests that fo-ti might inhibit CYP1A2. Additionally, in vitro research suggests that the degree of CYP1A2 inhibition depends on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, in an animal study, an aqueous extract of fo-ti inhibited CYP1A2 while an alcoholic extract of fo-ti induced CYP1A2. Induction or inhibition of CYP1A2 by fo-ti has not been reported in humans.

Your body relies on a liver enzyme called CYP1A2 to break down pirfenidone (Esbriet). Fo-ti may bump into that same enzyme. In test-tube studies, raw fo-ti seemed to slow CYP1A2 down, while in one animal study an alcohol-based extract sped it up instead. So the direction could go either way depending on the product.

If fo-ti slows the enzyme, pirfenidone could build up and you might notice more nausea, stomach upset, tiredness, dizziness, or sun sensitivity. If it speeds things up, the drug could clear faster and work less well. Keep in mind this is only lab and animal data, not seen in people, so it's uncertain. Since fo-ti has also been tied to liver injury and pirfenidone needs liver monitoring, please check with your pharmacist or doctor before combining them.

Pirfenidone — available as capsules and tablets under the brand name Esbriet, as well as generic pirfenidone products — is indicated for the treatment of idiopathic pulmonary fibrosis (IPF), a chronic, progressive disease in which scar tissue builds up in the lungs and makes breathing increasingly difficult.

Esbriet capsules and tablets, pirfenidone capsules, and pirfenidone tablets are each specifically approved for IPF. No other uses are supported by the available product labels.
