Thioridazine falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Fo-ti interacts with.

Theoretically, fo-ti may increase the levels and clinical effects of drugs metabolized by CYP2C19.
Animal and in vitro research suggests that fo-ti may inhibit CYP2C19. An in vitro study suggests that the degree of CYP2C19 inhibition may depend on the type of fo-ti extract (i.e., the raw plant leads to greater inhibition than extensively processed extracts). However, this interaction has not been reported in humans.

Here's the concern with pairing Fo-ti and thioridazine. Your body uses a liver enzyme called CYP2C19 to help clear thioridazine. Lab and animal studies hint that Fo-ti may slow that enzyme down, which could let more thioridazine build up than usual. Since thioridazine is an active drug (not a prodrug), more in your system means stronger effects and a higher chance of side effects like drowsiness, dizziness, low blood pressure, and especially changes in your heart rhythm (QT prolongation), which this medicine is already known for.

The good news: this is theoretical and has not been seen in real people yet. Still, because thioridazine's heart risks are serious, please check with your pharmacist or doctor before combining the two.

Here's the concern with this pairing. Your body relies on a liver enzyme called CYP2D6 to break down thioridazine and clear it out. Lab and animal studies hint that Fo-ti (especially the raw, less-processed root) may slow that enzyme down. If it does, thioridazine could build up in your system, which may mean stronger effects and more side effects like drowsiness, dizziness, low blood pressure, or a fast or irregular heartbeat.

That last point matters most. Thioridazine is known for affecting the heart's rhythm (QT prolongation), so even modest increases in its level deserve respect. The good news: this is theoretical and based only on lab and animal data, not real human reports. Still, please check with your pharmacist or doctor before combining the two.

What Is Thioridazine Used For?

Brand name: Mellaril

Thioridazine Hydrochloride tablets are used for the management of schizophrenia — but only in patients who have already tried at least two other antipsychotic medications without adequate results, either because those drugs didn't work well enough or caused intolerable side effects.

Because thioridazine carries serious heart-rhythm risks, it is considered a last-resort option for schizophrenia, not a first-line treatment. It is not approved for dementia-related psychosis or any other psychiatric condition.
