Ginger interacts with whole categories of medication. Acetaminophen, Chlorpheniramine, Codeine, Phenylephrine falls into 2 of those categories, so it can interact with Ginger in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Acetaminophen, Chlorpheniramine, Codeine, Phenylephrine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ginger interacts with.
In vitro research and some case reports suggest that ginger inhibits CYP3A4 activity. Three case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking ginger and cancer medications that are CYP3A4 substrates (imatinib, dabrafenib, and crizotinib). However, the causality of this interaction is unclear due to the presence of multiple interacting drugs and routes of administration.
Conversely, other in vitro research suggests that ginger induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. However, this interaction has not been reported in humans.

This combination product (Colrex) has several ingredients, and the one that interacts here through CYP3A4 is the codeine. Codeine is a bit unusual: it's mostly a prodrug, meaning your body has to convert it into morphine to relieve cough and pain. The bigger conversion step uses a different enzyme (CYP2D6), while CYP3A4 mostly shuttles codeine into an inactive byproduct.
The honest answer is that the evidence here is weak. It comes from lab and animal studies, and ginger has been shown to both block and speed up CYP3A4, so we can't even say which way it would push things in real life. In normal food and tea amounts, ginger is very unlikely to matter. If you take concentrated ginger supplements, just mention it to your pharmacist, especially watching for extra drowsiness or constipation from the codeine.

CYTOCHROME P450 1A2 (CYP1A2) SUBSTRATES

In vitro research shows that ginger inhibits CYP1A2 activity. However, this interaction has not been reported in humans.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
This combination product (Colrex) has four ingredients, and the only one tied to the lab finding here is the way ginger might mildly slow a liver enzyme called CYP1A2. The honest truth is this connection comes only from test-tube studies, and it has never actually been seen in real people. So I wouldn't lose sleep over it.
If ginger did nudge this enzyme, it could in theory let small amounts of these ingredients linger a bit longer, meaning slightly more drowsiness from the chlorpheniramine and codeine, or a touch more dry mouth. That's very unlikely with normal food or even typical ginger supplement doses. Still, if you take ginger capsules regularly, it never hurts to mention it to your pharmacist.
