Acetaminophen, Dextromethorphan, Phenylpropanolamine, Pyrilamine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Ginger interacts with.

Ginger might increase or decrease the levels of CYP3A4 substrates.
In vitro research and some case reports suggest that ginger inhibits CYP3A4 activity. Three case reports from the World Health Organization (WHO) adverse drug reaction database describe increased toxicity in patients taking ginger and cancer medications that are CYP3A4 substrates (imatinib, dabrafenib, and crizotinib). However, the causality of this interaction is unclear due to the presence of multiple interacting drugs and routes of administration.
Conversely, other in vitro research suggests that ginger induces CYP3A4 activity, leading to reduced levels of CYP3A4 substrates. However, this interaction has not been reported in humans.

This combination product, Theracaps, contains several ingredients (including dextromethorphan, the cough suppressant) that the liver helps break down using an enzyme called CYP3A4. Ginger has been studied in lab dishes and animals, and the findings are mixed. Some research hints that ginger may slow this enzyme a little, while other research suggests it may speed it up. Honestly, the evidence here is weak and has not been clearly shown in real people.

What does that mean for you? In theory, if ginger slowed things down, you might feel a bit more of dextromethorphan's effects (like drowsiness or dizziness). If it sped things up, the cough relief could be slightly weaker. The everyday culinary amount of ginger in food is very unlikely to cause any noticeable problem. If you regularly take concentrated ginger supplements, just mention it to your pharmacist or doctor to be safe.

Theracaps is a combination cold and cough product, and this flag is about the enzyme called CYP1A2. In lab dishes, ginger appears to slow this enzyme down a little. In theory, that could let some ingredients clear more slowly and build up slightly, which might mean a touch more drowsiness, dizziness, or a jittery feeling from the decongestant.

Here is the important part: this has only been seen in test-tube and animal studies, never in actual people. The interaction is rated minor, and everyday ginger in food or tea is very unlikely to cause any real problem. Concentrated ginger capsules are still no big worry here, but if you take them regularly, it never hurts to run it by your pharmacist or doctor first.
