ORAL DRUGS
Why this applies to Iothalamate
Iothalamate falls under ORAL DRUGS — the group Glucomannan interacts with.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing about Iothalamate (Conray): it's a contrast dye used to make certain X-ray studies clearer, and most of the time it's given as an injection by your medical team, not swallowed at home. Because glucomannan's gel-forming effect only changes how things are absorbed from your stomach and gut, it wouldn't have a meaningful effect on an injected dose.
If you were ever given an oral form of this contrast, the concern would be that glucomannan's thick fiber gel could slow or reduce how much dye your body takes up, which might make the imaging less reliable. This is a theoretical worry based mostly on other oral drugs. To be safe, separate them by taking glucomannan about 30 to 60 minutes after, and let your pharmacist or radiology team know what you take.
