Why this applies to Fenofibrate

Fenofibrate falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the simple version: glucomannan is a thick, gel-forming fiber, and when it's sitting in your stomach and intestines at the same time as your fenofibrate, it can grab onto the medicine and keep some of it from being absorbed. Less absorbed means a weaker cholesterol and triglyceride lowering effect over time, which could show up as numbers that don't improve the way you'd expect on your next lab check.

Fenofibrate is the active drug itself, so this isn't a prodrug situation. The fix is easy: separate the timing. A good habit is to take your fenofibrate first, then wait about 30 to 60 minutes before your glucomannan, and always drink plenty of water with the fiber. This concern is fairly modest and based on limited studies, but spacing them out is smart. Check with your pharmacist or doctor before changing anything.

It is generally well tolerated when taken with plenty of water, but it must be taken carefully because it can swell and cause choking or blockage if not swallowed properly. What Is Fenofibrate Used For?

Brand names: Tricor, Fenofibrate, Lipidil Micro, Lipidil Supra, Antara, Lofibra, Triglide, Lipidil EZ

Fenofibrate is used along with diet to improve blood fats in adults. Tricor and the fenofibrate tablets are used to lower LDL (“bad”) cholesterol, total cholesterol, triglycerides and apolipoprotein B, and to raise HDL (“good”) cholesterol in primary hypercholesterolemia (high cholesterol not caused by another condition) or mixed dyslipidemia. They are also used for severe hypertriglyceridemia (very high triglycerides).

Antara and Lipofen capsules are used to lower triglycerides in adults with severe hypertriglyceridemia (500 mg/dL or higher). They are also used to lower LDL cholesterol in primary hyperlipidemia (high blood fats not caused by another condition) when the recommended LDL-lowering therapy is not possible. Fenofibrate did not reduce heart disease complications or death in people with type 2 diabetes in large trials.
