Sweet Clover and Fenofibrate

Here is the interaction we found between these two, with the evidence behind it.

Moderate

HEPATOTOXIC DRUGS

Why this applies to Fenofibrate

Fenofibrate falls under HEPATOTOXIC DRUGS — the group Sweet Clover interacts with.

Theoretically, concomitant use of sweet clover with hepatoxic drugs might increase the risk of hepatotoxicity.
Orally, large amounts of sweet clover can cause transient liver injury in susceptible individuals, possibly due to coumarin content. In one case report, a patient with multiple sclerosis developed jaundiced palms and a slight elevation in serum alanine aminotransferase (ALT) after taking sweet clover containing coumarin 10 mg daily for 3 years. Two weeks after starting therapy with subcutaneous interferon (IFN)-beta 1b, aspartate transaminase (AST) and ALT levels increased to 7 and 17 times the upper limit of normal, respectively. Levels normalized after discontinuation of both products. Subsequent use of IFN-beta 1a alone, without sweet clover, resulted in only a very slight increase in AST.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the connection worth knowing about. Fenofibrate is a cholesterol and triglyceride medication that, on its own, can occasionally stress the liver and raise your liver enzyme levels. Sweet clover contains coumarin, which in large or concentrated amounts has also been linked to mild, usually reversible liver irritation. Put the two together and the worry is simply that you might be adding one liver stressor on top of another.

I want to be honest that this is a theoretical concern based on how each acts, not proven by solid studies, so there's no need to panic. Still, since your doctor likely already checks liver enzymes while you're on fenofibrate, mention any sweet clover supplement to your pharmacist or doctor first. Watch for yellowing skin or eyes, dark urine, or unusual fatigue, and report those right away.

From our pharmacist’s overview on the Fenofibrate page

“Fenofibrate (Tricor, Antara) is a cholesterol and triglyceride medicine that can strain the liver on its own, so this list of 59 supplements deserves genuine care rather than a shrug. Most of it, 38 ingredients, involves herbs that also stress the liver, ashwagandha among them, and stacking those effects is the risk that matters most h…”

What Is Fenofibrate Used For?

Brand names: Tricor, Fenofibrate, Lipidil Micro, Lipidil Supra, Antara, Lofibra, Triglide, Lipidil EZ

Fenofibrate is used alongside a cholesterol-lowering diet in adults to improve abnormal lipid levels. Tricor (tablet) and fenofibrate tablets are indicated to lower LDL cholesterol (bad cholesterol), total cholesterol, triglycerides, and apolipoprotein B, and to raise HDL cholesterol (good cholesterol) in adults with primary hypercholesterolemia (high cholesterol) or mixed dyslipidemia (mixed abnormal lipid levels). They are also used to treat severe hypertriglyceridemia (very high triglycerides).

Antara and Lipofen (capsules) are indicated to reduce triglyceride levels in adults with severe hypertriglyceridemia, and to lower LDL cholesterol in adults with primary hyperlipidemia when other recommended LDL-lowering therapies cannot be used. Fenofibrate has not been shown to reduce heart attacks or cardiovascular deaths in patients with type 2 diabetes.
