Why this applies to Allopurinol

Allopurinol falls under HEPATOTOXIC DRUGS — the group Sweet Clover interacts with.

Theoretically, concomitant use of sweet clover with hepatoxic drugs might increase the risk of hepatotoxicity.
Orally, large amounts of sweet clover can cause transient liver injury in susceptible individuals, possibly due to coumarin content. In one case report, a patient with multiple sclerosis developed jaundiced palms and a slight elevation in serum alanine aminotransferase (ALT) after taking sweet clover containing coumarin 10 mg daily for 3 years. Two weeks after starting therapy with subcutaneous interferon (IFN)-beta 1b, aspartate transaminase (AST) and ALT levels increased to 7 and 17 times the upper limit of normal, respectively. Levels normalized after discontinuation of both products. Subsequent use of IFN-beta 1a alone, without sweet clover, resulted in only a very slight increase in AST.

Here's the gist: sweet clover contains coumarin, and in large or concentrated doses it can occasionally stress the liver. Allopurinol, on its own, can also irritate the liver in some people, which is why doctors sometimes check liver enzymes when you start it. Stack the two together and, at least in theory, you could be adding two small liver burdens on top of each other.

I'll be honest with you, this is a theoretical concern based on how these substances behave, not proven cases of the combo causing harm. Allopurinol is not a prodrug here, so the worry isn't about changing how well it works for your gout. Watch for things like unusual tiredness, yellowing of the skin or eyes, dark urine, or stomach pain, and please run this by your pharmacist or doctor before combining them.

About Allopurinol

What Is Allopurinol Used For?

Brand names: Zyloprim, Caplenal, Cosuric, Rimapurinol, Zyloric

Allopurinol tablets (including Zyloprim) are used in adults to treat the signs and symptoms of gout — including gout attacks, tophi (uric acid deposits), joint damage, and kidney problems caused by uric acid. Allopurinol tablets are also used in adults to reduce recurrent calcium oxalate kidney stones when uric acid in the urine is too high, even after lifestyle changes.

For adults and children with leukemia, lymphoma, or solid tumor cancers, allopurinol is used to control uric acid levels that rise during cancer treatment. The injectable forms — Allopurinol sodium for injection and Aloprim — are specifically for patients who cannot take medicine by mouth and are given intravenously (through a vein) in a hospital or clinical setting. Note: allopurinol tablets are not recommended for people who have high uric acid levels but no symptoms.
