Sweet Clover and Dantrolene

Here is the interaction we found between these two, with the evidence behind it.

Moderate

HEPATOTOXIC DRUGS

Why this applies to Dantrolene

Dantrolene falls under HEPATOTOXIC DRUGS — the group Sweet Clover interacts with.

Theoretically, concomitant use of sweet clover with hepatoxic drugs might increase the risk of hepatotoxicity.
Orally, large amounts of sweet clover can cause transient liver injury in susceptible individuals, possibly due to coumarin content. In one case report, a patient with multiple sclerosis developed jaundiced palms and a slight elevation in serum alanine aminotransferase (ALT) after taking sweet clover containing coumarin 10 mg daily for 3 years. Two weeks after starting therapy with subcutaneous interferon (IFN)-beta 1b, aspartate transaminase (AST) and ALT levels increased to 7 and 17 times the upper limit of normal, respectively. Levels normalized after discontinuation of both products. Subsequent use of IFN-beta 1a alone, without sweet clover, resulted in only a very slight increase in AST.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the thing about dantrolene: it's already known to be tough on the liver. In fact, it carries one of the stronger liver-injury warnings of any muscle relaxant. Sweet clover contains coumarin, which in higher supplement doses has also been linked to liver irritation in sensitive people. So the worry here isn't a chemical clash, it's that you'd be putting two liver stressors together at the same time.

This concern is theoretical, based on how each one behaves rather than studies pairing them directly, so I don't want to alarm you. Dantrolene isn't a prodrug, so we're not talking about it working harder or weaker. Still, watch for yellowing skin or eyes, dark urine, belly pain, or unusual tiredness, and please check with your doctor or pharmacist before combining them.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).

  • Leg swelling (edema)
  • Hemorrhoids
  • Bruising and minor inflammation

About Dantrolene

What Is Dantrolene Used For?

Brand names: Dantrium, Dantrium Intravenous

Dantrolene sodium capsules (Dantrium, Dantrolene Sodium) are used by mouth to control spasticity — painful or disabling muscle stiffness and spasms — caused by upper motor neuron disorders such as spinal cord injury, stroke, cerebral palsy, or multiple sclerosis. They are not used for muscle spasm from arthritis or other rheumatic conditions. Oral capsules are also used before and after surgery to prevent or reduce the risk of malignant hyperthermia in patients known to be susceptible.

For an active malignant hyperthermia crisis — a sudden, dangerous overheating of the muscles triggered by certain anesthetics — the intravenous (IV) forms are given in the hospital. Dantrium Intravenous, Revonto, Ryanodex, and Dantrolene injection are all indicated to treat a malignant hyperthermia crisis and may also be given before surgery to help prevent one in high-risk patients.
