Why this applies to Fingolimod

Fingolimod falls under HEPATOTOXIC DRUGS — the group Sweet Clover interacts with.

Theoretically, concomitant use of sweet clover with hepatoxic drugs might increase the risk of hepatotoxicity.
Orally, large amounts of sweet clover can cause transient liver injury in susceptible individuals, possibly due to coumarin content. In one case report, a patient with multiple sclerosis developed jaundiced palms and a slight elevation in serum alanine aminotransferase (ALT) after taking sweet clover containing coumarin 10 mg daily for 3 years. Two weeks after starting therapy with subcutaneous interferon (IFN)-beta 1b, aspartate transaminase (AST) and ALT levels increased to 7 and 17 times the upper limit of normal, respectively. Levels normalized after discontinuation of both products. Subsequent use of IFN-beta 1a alone, without sweet clover, resulted in only a very slight increase in AST.

Here's the concern with pairing sweet clover and fingolimod (Gilenya, Tascenso ODT). Both can be hard on the liver. Fingolimod itself can raise your liver enzymes, and sweet clover's coumarin has been linked to liver irritation in some people, especially in concentrated supplement doses or if the plant has spoiled. Put them together and you may stack two stressors on the same organ.

This is a theoretical caution based on how each one behaves, not proven cause and effect, so I don't want to alarm you. Still, watch for yellowing of the skin or eyes, dark urine, belly pain, or unusual tiredness. Since fingolimod already requires liver monitoring, please check with your doctor or pharmacist before adding sweet clover.

What Is Fingolimod Used For?

Brand names: Gilenya, Tascenso ODT

Fingolimod — available as capsules (Fingolimod, Fingolimod Hydrochloride, and Gilenya) and as an orally disintegrating tablet (Tascenso ODT) — is approved for the treatment of relapsing forms of multiple sclerosis (MS) in patients 10 years of age and older. This includes clinically isolated syndrome (a first MS episode), relapsing-remitting MS (the most common type with flare-ups and periods of stability), and active secondary progressive MS (a later stage where disability gradually worsens but relapses still occur).

All supported formulations share these same approved uses. The goal of treatment is to reduce the frequency of relapses and slow disease activity.
