Why this applies to Quinidine

Quinidine falls under HEPATOTOXIC DRUGS — the group Sweet Clover interacts with.

Theoretically, concomitant use of sweet clover with hepatoxic drugs might increase the risk of hepatotoxicity.
Orally, large amounts of sweet clover can cause transient liver injury in susceptible individuals, possibly due to coumarin content. In one case report, a patient with multiple sclerosis developed jaundiced palms and a slight elevation in serum alanine aminotransferase (ALT) after taking sweet clover containing coumarin 10 mg daily for 3 years. Two weeks after starting therapy with subcutaneous interferon (IFN)-beta 1b, aspartate transaminase (AST) and ALT levels increased to 7 and 17 times the upper limit of normal, respectively. Levels normalized after discontinuation of both products. Subsequent use of IFN-beta 1a alone, without sweet clover, resulted in only a very slight increase in AST.

Here's the simple version. Both sweet clover and quinidine can put a little stress on your liver. Quinidine, on its own, occasionally bumps up liver enzymes or, rarely, causes hepatitis. Sweet clover contains coumarin, which in large or spoiled doses has been linked to mild, usually temporary liver irritation. Stacking two things that lean on the liver could, in theory, add up.

I want to be honest about the evidence though: this is a theoretical concern based on how these things behave, not a proven, common problem. Quinidine is not a prodrug, so this isn't about it working harder or weaker, it's just an overlapping liver-strain idea. Culinary pinches of sweet clover are unlikely to matter, but concentrated supplements are a different story. Please check with your pharmacist or doctor before combining them.

About Quinidine

What Is Quinidine Used For?

Brand names: Quinaglute Duratabs, Quinaglute, Duraquin, Cardioquin, Cin-Quin, Quinora, Lin-Qin, Quinova, Quinidex Extentabs

Quinidine is used to help restore a normal heart rhythm in people with atrial fibrillation or atrial flutter (irregular, rapid heart rhythms originating in the upper chambers). It is also used on an ongoing basis to reduce how often those episodes come back, in patients whose episodes are frequent and poorly tolerated — but only after other options have been tried first.

Quinidine sulfate oral tablets and quinidine gluconate extended-release oral tablets are both approved for these heart-rhythm uses. Quinidine sulfate tablets are additionally used as part of an approved treatment regimen for life-threatening Plasmodium falciparum malaria, in combination with quinidine gluconate injection (the central component of that regimen). Both formulations may also be used to suppress recurrent, documented life-threatening ventricular arrhythmias (dangerous rapid rhythms in the lower chambers of the heart), such as sustained ventricular tachycardia.
