Why this applies to Riluzole

Riluzole falls under HEPATOTOXIC DRUGS — the group Sweet Clover interacts with.

Theoretically, concomitant use of sweet clover with hepatoxic drugs might increase the risk of hepatotoxicity.
Orally, large amounts of sweet clover can cause transient liver injury in susceptible individuals, possibly due to coumarin content. In one case report, a patient with multiple sclerosis developed jaundiced palms and a slight elevation in serum alanine aminotransferase (ALT) after taking sweet clover containing coumarin 10 mg daily for 3 years. Two weeks after starting therapy with subcutaneous interferon (IFN)-beta 1b, aspartate transaminase (AST) and ALT levels increased to 7 and 17 times the upper limit of normal, respectively. Levels normalized after discontinuation of both products. Subsequent use of IFN-beta 1a alone, without sweet clover, resulted in only a very slight increase in AST.

Here's the thing to keep in mind with riluzole: it's already known to be tough on the liver, and your doctor likely checks your liver blood tests (ALT and AST) regularly while you're on it. Sweet clover contains coumarin, which in large or concentrated supplement amounts has been linked to liver irritation in some people. So the worry here is simply additive, two things leaning on the liver at the same time could raise your odds of liver inflammation.

I want to be honest that this is a theoretical concern based on how these substances behave, not strong proof. Still, with riluzole I'd play it safe. Watch for yellowing of the skin or eyes, dark urine, belly pain, or unusual tiredness, and please talk with your pharmacist or doctor before combining them.

Riluzole is approved to treat amyotrophic lateral sclerosis (ALS) — the progressive neurological disease also known as Lou Gehrig's disease. This indication is shared across all available riluzole products, including tablets (Rilutek, riluzole), the dissolving oral film (Exservan), and the oral suspension (Tiglutik).

Tiglutik oral suspension is notable in that it can also be given through a PEG-tube (percutaneous endoscopic gastrostomy tube) for patients who have difficulty swallowing.
