Isavuconazonium Sulfate (oral) falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the thing to keep in mind with Cresemba (isavuconazonium): glucomannan is a thick soluble fiber that turns into a gel in your gut, and that gel can slow or reduce how well some pills are absorbed. The worry is that taking them close together could lower the amount of antifungal that actually gets into your bloodstream. With a serious infection like aspergillosis or mucormycosis, you really want steady, reliable drug levels, so even a modest dip in absorption is worth avoiding.

The evidence here is fairly general (a small human study on a different drug plus some animal data), so this is more a sensible precaution than a proven problem for Cresemba specifically. A simple fix is spacing: take your medicine first, then take glucomannan about 30 to 60 minutes later. Please run this by your pharmacist or doctor before combining them.

What Is Isavuconazonium Used For?

Brand name: Cresemba

Cresemba (isavuconazonium sulfate) is used to treat two serious, life-threatening fungal infections: invasive aspergillosis and invasive mucormycosis. These infections can spread through the body and are most dangerous in people with weakened immune systems.

Cresemba for injection is approved for adults and children 1 year of age and older. Cresemba capsules are approved for adults and children 6 years of age and older who weigh at least 16 kilograms (about 35 pounds).
