Isosorbide Dinitrate falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the gist with glucomannan and isosorbide dinitrate. Glucomannan is a thick, water-loving fiber that forms a gel in your stomach. That gel can trap or slow down pills swallowed around the same time, so less of your isosorbide may actually get absorbed. This isn't about your liver enzymes, it's simply a physical mixing-and-absorption issue. The concern is mostly theoretical for this drug, drawn from studies on other oral medicines, but the worry is real enough to plan around.

If absorption drops, your isosorbide might not work as well, meaning less protection against angina (chest pain). A simple fix usually helps: take your isosorbide first, then wait about 30 to 60 minutes before your glucomannan. Please run this timing past your pharmacist or doctor before changing anything.

What Is Isosorbide Used For?

Brand names: Isordil, Angitak, Cedocard Retard, Isoket Retard, Isoket

Isosorbide is used to prevent angina pectoris — chest pain or tightness caused by coronary artery disease. Both isosorbide dinitrate tablets and isosorbide mononitrate tablets (including Monoket and Isordil) are used for this purpose.

Important: none of these oral formulations work fast enough to stop a chest pain episode that has already started. They are taken on a schedule to prevent angina, not to treat it in the moment.
