Why this applies to Letermovir (Oral)

Letermovir (Oral) falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the simple version: glucomannan is a fiber that turns into a thick gel in your gut. That gel can grab onto pills you swallow and slow down how much of them your body actually absorbs. With Letermovir (Prevymis), the worry is that less of the drug gets into your bloodstream, which could mean weaker protection against CMV at a time when you really need it after a stem-cell transplant.

This concern is mostly theoretical and based on studies of other oral drugs, not letermovir itself, so we can't say exactly how big the effect would be. Still, given how important this medicine is, it's worth being careful. The easy fix is timing: take your letermovir first, then wait about 30 to 60 minutes before the glucomannan. Please run this by your transplant team or pharmacist before mixing the two.

What Is Letermovir Injection Used For?

Brand name: Prevymis

Prevymis injection is used to prevent cytomegalovirus (CMV) infection and disease in adults and children 6 months of age and older (weighing at least 6 kg) who have received an allogeneic hematopoietic stem cell transplant (HSCT) and are CMV-seropositive (meaning they carry the CMV virus).

Prevymis injection is also used to prevent CMV disease in adults and children 12 years of age and older (weighing at least 40 kg) who have received a kidney transplant and are at high risk — specifically when the donor tested positive for CMV but the recipient tested negative (D+/R-).
