Why this applies to Mefloquine

Mefloquine falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the simple version: glucomannan is a thick soluble fiber that turns into a gel in your gut. That gel can trap part of an oral medicine and keep it from being fully absorbed. The worry with mefloquine is that you might soak up a little less of your dose, which could leave you with weaker protection against malaria. That matters, because with malaria prevention you really want the full, reliable amount in your system.

The good news is the fix is mostly about timing. The general advice is to take your mefloquine first and wait about 30 to 60 minutes before taking glucomannan with plenty of water. This interaction is based on limited human and animal data, so it's not proven for mefloquine specifically. Before traveling, run your full routine past your pharmacist or doctor.

Glucomannan is a soluble fiber from the konjac root that swells into a gel in your stomach, which may help with mild weight loss, constipation, and modestly lowering cholesterol. It is generally well tolerated when taken with plenty of water, but it must be taken carefully because it can swell and cause choking or blockage if not swallowed properly. About Mefloquine

What Is Mefloquine Used For?

Brand name: Lariam

Mefloquine Hydrochloride tablets are used to treat mild to moderate malaria caused by P. falciparum (including strains resistant to chloroquine) or P. vivax. They are also used to prevent malaria caused by these two parasite species, including chloroquine-resistant strains of P. falciparum, in people traveling to or living in areas where malaria is common.

Important note: Mefloquine does not eliminate the liver-stage parasites of P. vivax, so patients treated for P. vivax malaria will need a follow-up medication (such as primaquine) to prevent the infection from coming back. For life-threatening or severe malaria, an intravenous antimalarial is used first; mefloquine may then be given to complete the course.
