Why this applies to Pegaspargase

Pegaspargase falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's something worth clearing up: this fiber interaction is really about oral medicines that need to be absorbed from your gut. Pegaspargase (Oncaspar) doesn't fit that picture. It's given as an injection or infusion under the care of your cancer team, so it never passes through your stomach where glucomannan's thick gel could slow things down.

Because of that, I wouldn't expect glucomannan to change how well your pegaspargase works. The bigger thing to know is that during ALL chemotherapy, your care team usually wants to approve any supplement first, since fiber can affect other oral pills you take and your overall nutrition. So please run glucomannan by your oncologist or pharmacist before starting it.

Glucomannan is a soluble fiber from the konjac root that swells into a gel in your stomach, which may help with mild weight loss, constipation, and modestly lowering cholesterol. It is generally well tolerated when taken with plenty of water, but it must be taken carefully because it can swell and cause choking or blockage if not swallowed properly. What Is Pegaspargase Injection Used For?

Brand name: Oncaspar

Oncaspar (pegaspargase) injection is used as part of a multi-drug chemotherapy regimen to treat acute lymphoblastic leukemia (ALL) — a fast-growing cancer of the blood and bone marrow — in both pediatric and adult patients. It is approved for first-line (initial) treatment of ALL.

Oncaspar is also specifically approved for patients with ALL who have had a serious allergic reaction (hypersensitivity) to native forms of L-asparaginase, offering an option when other asparaginase products can no longer be used safely.
