Why this applies to Pilocarpine

Pilocarpine falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the thing to keep in mind when you take pilocarpine by mouth (like Salagen) alongside glucomannan. This fiber swells into a thick gel in your stomach and gut, and that gel can trap part of the pilocarpine before your body absorbs it. The worry is that you'd get less of the medicine into your bloodstream, so it may not work as well at relieving your dry mouth. You might simply notice your saliva flow isn't quite what you expected.

The good news is the fix is easy and timing-based. Take your pilocarpine first, then wait about 30 to 60 minutes before your glucomannan. This is moderate, fairly likely concern, but mostly avoidable. If you take these regularly, just run your schedule by your pharmacist or doctor.

Glucomannan is a soluble fiber from the konjac root that swells into a gel in your stomach, which may help with mild weight loss, constipation, and modestly lowering cholesterol. It is generally well tolerated when taken with plenty of water, but it must be taken carefully because it can swell and cause choking or blockage if not swallowed properly. About Pilocarpine

What Is Pilocarpine Used For?

Brand names: Salagen, Pilocar, Vuity, Qlosi

Pilocarpine eye drops are used to lower elevated pressure inside the eye in people with open-angle glaucoma or ocular hypertension, to manage acute angle-closure glaucoma, to prevent pressure spikes after laser eye surgery, and to cause pupil constriction (miosis) when needed. Vuity and Qlosi eye drops, as well as Pilocarpine Hydrochloride Ophthalmic solution, are specifically approved to treat presbyopia (age-related difficulty focusing up close) in adults.

Pilocarpine tablets — including Salagen and generic pilocarpine hydrochloride tablets — are used by mouth to relieve dry mouth caused by salivary gland damage from radiation therapy for head and neck cancer, or from Sjögren's syndrome (an autoimmune condition that reduces saliva and tear production).
