Why this applies to Sirolimus

Sirolimus falls under ORAL DRUGS — the group Glucomannan interacts with.

Theoretically, glucomannan may decrease absorption of drugs taken orally.
Due to its viscosity and bulking effects, there is concern that glucomannan can decrease the absorption of oral drugs. A small clinical study in healthy volunteers shows that taking glyburide 2.5 mg plus glucomannan 3.9 grams with breakfast reduces plasma levels of glyburide when compared with breakfast and glyburide alone. In addition, animal research demonstrates this effect on amoxicillin, but shows increased absorption of metronidazole. This mouse model also demonstrates that metronidazole elimination is prolonged, but amoxicillin elimination is enhanced by 38%; glucomannan may also affect the distribution of some drugs. To avoid changes in absorption, take glucomannan 30-60 minutes after taking oral drugs.

Here's the concern with pairing glucomannan and sirolimus. Glucomannan is a soluble fiber that forms a thick gel in your gut, and that gel can physically slow or block how well some oral medicines get absorbed. Sirolimus isn't a prodrug, so this isn't about activating it. The worry is simpler: the fiber gel could trap some of your dose and lower the amount that reaches your bloodstream.

That matters a lot here because sirolimus is a narrow-therapeutic-index transplant medicine. Even small drops in blood levels could weaken the protection it gives your transplanted kidney, raising the risk of rejection. The evidence comes from related drugs, not sirolimus directly, so it's somewhat theoretical. Still, with a drug this important, please talk with your transplant team or pharmacist before starting glucomannan, and never adjust your sirolimus on your own.

What Is Sirolimus Used For?

Brand name: Rapamune

Sirolimus oral solution and tablets are used to help prevent organ rejection in kidney (renal) transplant patients aged 13 and older. They are also used to treat lymphangioleiomyomatosis (LAM) — a rare lung disease in which abnormal muscle-like cells invade lung tissue.

Fyarro (sirolimus, intravenous injection) is used to treat adults with locally advanced, unresectable, or metastatic malignant PEComa — a rare tumor that grows in soft tissue or organs. Hyftor (sirolimus topical gel) is used to treat facial angiofibroma (small benign skin growths on the face) associated with tuberous sclerosis in adults and children aged 6 and older.
