Alvimopan falls under P-GLYCOPROTEIN SUBSTRATES — the group Hesperidin interacts with.

Theoretically, hesperidin might inhibit P-glycoprotein-mediated drug efflux and potentially increase levels of drugs that are substrates of P-glycoprotein.
In vitro research shows that hesperidin can inhibit P-glycoprotein efflux. This effect has not been reported in humans.

Here's the gist for Entereg (alvimopan). Your body uses a tiny pump called P-glycoprotein to push certain drugs back out of your cells and your gut. Hesperidin, the citrus flavonoid, has been shown in lab dish studies only to slow that pump down. In theory, that could let a little more alvimopan stay in your system than expected.

I want to be honest about how soft this evidence is. It has only been seen in test tubes, never confirmed in actual people, so the real-world risk is likely small. Alvimopan is also a short-term medicine used right after bowel surgery in a hospital setting. Still, mention any hesperidin or citrus-flavonoid supplements to your surgical team or pharmacist so they can keep an eye on things.

Hesperidin is a flavonoid found in citrus fruits that is often combined with diosmin and used for vein and circulation problems like hemorrhoids and varicose veins. Some evidence supports these uses, but data for many other claims is limited. It is generally well tolerated, but pregnant and breastfeeding people should avoid it because safety has not been established.

What Is Alvimopan Used For?

Brand name: Entereg

Alvimopan capsules are used in hospitals to help the digestive tract recover more quickly after surgery that involves removing part of the bowel (partial bowel resection with primary anastomosis — meaning the remaining ends are reconnected). After this type of surgery, the bowel often temporarily stops working, a condition called postoperative ileus.

This medication is only given during a short hospital stay and is not for long-term or home use.
