Berotralstat Hydrochloride falls under P-GLYCOPROTEIN SUBSTRATES — the group Hesperidin interacts with.

Theoretically, hesperidin might inhibit P-glycoprotein-mediated drug efflux and potentially increase levels of drugs that are substrates of P-glycoprotein.
In vitro research shows that hesperidin can inhibit P-glycoprotein efflux. This effect has not been reported in humans.

Here's the situation with Orladeyo (berotralstat), which people take to prevent hereditary angioedema attacks. Your body uses a kind of cellular bouncer called P-glycoprotein to push this drug back out of cells. Hesperidin, a citrus flavonoid, may slow that bouncer down. If it does, a bit more berotralstat could hang around, which in theory might nudge up side effects like headache, belly pain, diarrhea, nausea, or back pain.

The honest part: this is only seen in lab dishes, not in real people, so it's mostly theoretical. The occasional orange or glass of juice is very unlikely to matter. Concentrated hesperidin supplements are the bigger question mark, so just run them by your pharmacist or doctor before starting.
