Here's the thing with clodronate (Ostac): it's a bisphosphonate that your body already absorbs very poorly from the gut, even on a good day. That's why it has to be taken on an empty stomach, away from food, calcium, and other things that can bind it up. Karaya gum is a bulk-forming fiber that swells into a gel when it hits water, and that gel can physically trap pills and slow or reduce how much medicine actually gets into your bloodstream.

For a drug like clodronate, where absorption is already low, even a small drop could mean it works less well for your bones. This concern is theoretical, based on how fiber behaves rather than firm studies. To be safe, separate them by taking karaya gum 30 to 60 minutes after your clodronate, and check the timing with your pharmacist.

Karaya gum is a natural plant gum from the Sterculia tree that is mostly used as a bulk-forming laxative and as a thickener or adhesive in food, dental, and ostomy products. As a fiber-type laxative it may help with occasional constipation, but high-quality evidence is limited, and it should be taken with plenty of water to avoid choking or blockage. Talk to your pharmacist or doctor before using it, especially if you have swallowing problems or bowel conditions.
