Here's the gist with karaya gum and phentermine: karaya is a bulky fiber that swells into a gel in your gut. When that gel is sitting there at the same time as a swallowed pill, it can sometimes grab onto the medicine and slow or reduce how much gets absorbed. Phentermine is not a prodrug, so the concern here is simply that the fiber could blunt its absorption, meaning a little less appetite-suppressing effect.

Honestly, this is a theoretical, minor concern based on how fibers behave, not strong proof. To play it safe, the usual advice is to take your phentermine first, then wait about 30 to 60 minutes before the karaya gum (with plenty of water). If you notice your appetite control feels off, check in with your pharmacist or doctor.

Karaya gum is a natural plant gum from the Sterculia tree that is mostly used as a bulk-forming laxative and as a thickener or adhesive in food, dental, and ostomy products. As a fiber-type laxative it may help with occasional constipation, but high-quality evidence is limited, and it should be taken with plenty of water to avoid choking or blockage. Phentermine is used as a short-term add-on (a few weeks) to a weight-loss plan built on exercise, behavior changes and fewer calories. It is for managing exogenous obesity (obesity not caused by another medical condition).

It is meant for people with a starting body mass index (BMI) of 30 or higher. It can also be used at a BMI of 27 or higher when other risk factors are present, such as controlled high blood pressure, diabetes or high cholesterol or other blood fats (hyperlipidemia).
