Here's the practical concern with pairing karaya gum and your diltiazem. Karaya gum is a bulky fiber that swells into a gel in your gut. When you swallow a pill right alongside it, that gel can trap some of the medicine and slow or reduce how much gets absorbed into your bloodstream. With a blood pressure and chest pain medicine like diltiazem, you'd ideally want steady, predictable absorption.
The good news: this is a theoretical interaction based on how fiber behaves, not on strong studies, and it's rated minor. Diltiazem is not a prodrug, so the worry is simply that less might get absorbed, which could mean weaker blood pressure or angina control. An easy fix is spacing them out: take your diltiazem first, then the karaya gum about 30 to 60 minutes later. Check with your pharmacist if you're unsure.

Karaya gum is a natural plant gum from the Sterculia tree that is mostly used as a bulk-forming laxative and as a thickener or adhesive in food, dental, and ostomy products. As a fiber-type laxative it may help with occasional constipation, but high-quality evidence is limited, and it should be taken with plenty of water to avoid choking or blockage. Oral extended-release diltiazem (capsules and tablets) treats high blood pressure (hypertension). It is also used for chronic stable angina, which is chest pain from the heart working harder than its blood supply allows. It can be used alone or with other blood pressure medicines.
Some extended-release capsule products, such as Cartia, Cartia Xt and certain generic diltiazem capsules, are also approved for angina due to coronary artery spasm. The intravenous (IV) injection is a different use: it gives temporary control of a rapid heart rate in atrial fibrillation or atrial flutter, and rapidly converts paroxysmal supraventricular tachycardia (PSVT) to normal rhythm.
