Here's the simple version: Karaya gum is a fiber that swells into a gel in your gut. When you swallow a pill like isosorbide dinitrate around the same time, that gel can trap some of it and slow or reduce how much actually gets absorbed. This is a theoretical concern based on how fiber behaves, not something proven to cause problems with this specific medication, and it's rated minor.

If less of your isosorbide is absorbed, it might not control your chest pain (angina) as well. The easy fix is timing. Take your isosorbide first, then wait 30 to 60 minutes before your karaya gum. If you have angina that isn't well controlled, please check with your pharmacist or doctor before changing anything.

Karaya gum is a natural plant gum from the Sterculia tree that is mostly used as a bulk-forming laxative and as a thickener or adhesive in food, dental, and ostomy products. As a fiber-type laxative it may help with occasional constipation, but high-quality evidence is limited, and it should be taken with plenty of water to avoid choking or blockage. Talk to your pharmacist or doctor before using it, especially if you have swallowing problems or bowel conditions.

What Is Isosorbide Used For?

Brand names: Isordil, Angitak, Cedocard Retard, Isoket Retard, Isoket

Isosorbide is used to prevent angina pectoris — chest pain or tightness caused by coronary artery disease. Both isosorbide dinitrate tablets and isosorbide mononitrate tablets (including Monoket and Isordil) are used for this purpose.

Important: none of these oral formulations work fast enough to stop a chest pain episode that has already started. They are taken on a schedule to prevent angina, not to treat it in the moment.
