Iloprost falls under ANTIHYPERTENSIVE DRUGS — the group N-acetyl Cysteine (nac) interacts with.

Theoretically, N-acetyl cysteine might increase the risk of hypotension when taken with antihypertensive drugs.
Animal research suggests that N-acetyl cysteine potentiates the hypotensive effects of the angiotensin-converting enzyme inhibitors (ACEIs) captopril and enalaprilat. Theoretically, combining N-acetyl cysteine with other antihypertensive drugs might increase the risk of hypotension.

Here's the simple version: iloprost (Ventavis) already works by relaxing and widening your blood vessels, so it naturally lowers blood pressure. NAC may add a little extra blood-pressure-lowering effect on top of it. If your pressure drops too low, you might feel dizzy, lightheaded, faint, or get a headache or flushing, which are already common with iloprost.

I want to be honest about the strength of this concern, though. This is mostly theoretical and based on animal studies with blood pressure drugs, not human studies with iloprost. So it's worth knowing, not worth panicking over. If you take iloprost and want to start NAC, run it by your pharmacist or doctor first, and let them know if you notice new dizziness.

Theoretically, N-acetyl cysteine might increase the risk of bleeding when taken with anticoagulant or antiplatelet drugs.
Clinical research suggests that intravenous N-acetyl cysteine decreases prothrombin time, prolongs coagulation time, decreases platelet aggregation, and increases blood loss in surgical patients. Furthermore, in vitro research suggests that N-acetyl cysteine increases the anticoagulant activity of nitroglycerin.

Here's the thing to keep in mind with iloprost (Ventavis): besides relaxing blood vessels in your lungs, it also has a mild effect of making platelets less likely to clump together. NAC, in turn, has shown (mostly in higher intravenous doses and in surgical patients) that it can thin the blood a bit, lower platelet stickiness, and lengthen clotting times. Stacked together, the worry is a slightly higher chance of easy bruising, nosebleeds, or bleeding that's slower to stop.

I'll be honest, this concern is mostly theoretical and based on how these two behave, not on strong real-world studies with everyday oral NAC capsules. Still, since iloprost already nudges platelets, it's worth a quick chat with your doctor or pharmacist before you add NAC, especially if you notice any unusual bruising or bleeding.

Aurlumyn (iloprost injection) is FDA-approved to treat severe frostbite in adults. Specifically, it is used to reduce the risk of losing fingers or toes (digit amputation) after serious frostbite injury. The clinical evidence supporting this use comes from young, healthy adults who suffered frostbite at high altitudes.
