Rezafungin falls under HEPATOTOXIC DRUGS — the group Niacin interacts with.

Theoretically, concomitant use of niacin and hepatotoxic drugs might increase the risk of hepatotoxicity.
Niacin has been associated with cases of liver toxicity, especially when used in pharmacologic doses. Sustained-release niacin preparations appear to be associated with a higher risk of hepatotoxicity than immediate-release niacin.

Here's the concern in plain terms. Both high-dose niacin and Rezafungin can, on their own, put some stress on the liver. Rezafungin isn't a prodrug, so this isn't about niacin changing how well the antifungal works. Instead, the worry is simply doubling up on two things that can each irritate the liver, which could raise your odds of liver-related trouble.

I want to be honest that this is a theoretical caution based on how these drugs behave, not proven cases of the two together. Regular food or a normal daily vitamin B3 dose is very unlikely to matter. High-dose niacin (especially sustained-release forms) is where I'd pay attention. Since Rezafungin treats a serious bloodstream infection, please loop in your doctor or pharmacist before adding any niacin so they can watch your liver labs.

What Is Rezafungin Injection Used For?

Brand name: Rezzayo

Rezzayo (rezafungin injection) is used to treat candidemia (a Candida yeast infection in the bloodstream) and invasive candidiasis (a serious Candida infection that has spread into the body's tissues or organs). It is approved for adults and for adolescents 12 years of age and older who have limited or no other treatment options.

Important note: Rezzayo has not been studied in Candida infections of the heart (endocarditis), bones (osteomyelitis), or the brain and spinal cord (meningitis), so it is not used for those conditions.
