Why this applies to Teplizumab-mzwv
Teplizumab-mzwv falls under HEPATOTOXIC DRUGS — the group Niacin interacts with.
Niacin has been associated with cases of liver toxicity, especially when used in pharmacologic doses. Sustained-release niacin preparations appear to be associated with a higher risk of hepatotoxicity than immediate-release niacin.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the concern with pairing high-dose niacin and Tzield (teplizumab). Tzield is a short course infusion given to delay type 1 diabetes, and one of its known effects is that it can stress the liver, sometimes showing up as raised liver enzymes on bloodwork. Niacin, especially at the big cholesterol-lowering doses and particularly in sustained-release forms, can also be hard on the liver. So the worry is simply that two liver stressors at once may add up.
To be clear, this is a theoretical caution based on how both work, not something proven to happen. Normal vitamin-level niacin from food or a regular multivitamin is very unlikely to matter. If you take high-dose niacin, please mention it to your care team before your Tzield treatment so they can watch your liver labs.

What Is Teplizumab-mzwv injection Used For?
Brand name: Tzield
Tzield is used to delay the onset of Stage 3 type 1 diabetes (T1D) in adults and children aged 1 year and older who already have Stage 2 T1D — meaning their immune system is attacking insulin-producing cells, but they don't yet have overt high blood sugar symptoms.
Tzield is also used to slow the loss of the body's own insulin production in children and teenagers aged 8 to 17 years who have recently been diagnosed with Stage 3 T1D. This second use is approved under accelerated approval, meaning continued approval depends on results from ongoing confirmatory studies.
