Why this applies to Glipizide, Metformin

Glipizide, Metformin falls under ANTIDIABETES DRUGS — the group Oleic Acid interacts with.

Theoretically, oleic acid might increase the effects of antidiabetes drugs. Preliminary clinical research in patients with type 2 diabetes taking oral hypoglycemic drugs shows that eating a diet rich in oleic acid from olive oil decreases fasting blood glucose levels when compared to eating a diet rich in linoleic acid from sunflower oil. It is unknown if taking oleic acid supplements would have this effect or if this change is clinically significant. Until more is known, use caution. Dose adjustment may be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (DiaBeta, Glynase PresTab, Micronase), insulin, metformin (Glucophage), pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the practical takeaway for your Metaglip (the glipizide plus metformin combo). When some patients with type 2 diabetes swapped to a diet rich in oleic acid (think olive oil) instead of other fats, their fasting blood sugar dropped a bit more. So the concern is that oleic acid may gently add to what your medication is already doing, which means your blood sugar could run lower than usual. Since glipizide can cause true low blood sugar, watch for shakiness, sweating, dizziness, or confusion.

Honestly, this is mostly seen with olive oil as part of a Mediterranean-style diet, and we don't know whether concentrated oleic acid supplements act the same way or whether the change is big enough to matter. I wouldn't lose sleep over cooking with olive oil. If you're thinking about a supplement, just loop in your pharmacist or doctor first so they can keep an eye on your numbers.
