Here's the simple version: Remifentanil (Ultiva) is a powerful, fast-acting opioid given through an IV during surgery or procedures, and one of its main effects is calming down the brain and slowing breathing. The thought here is that berberine, a compound in Oregon grape, may have a mild sedative effect of its own. Stacked on top of remifentanil, it could in theory add to the drowsiness and deepen the slowdown in breathing.

I want to be honest about how thin this evidence is. It comes only from animal and lab studies, not from people, so it's more of a caution than a proven problem. Since remifentanil is only used in a monitored hospital setting, your care team controls the dose closely. Still, let your anesthesiologist or pharmacist know about any supplements you take, including Oregon grape.
