Why this applies to Colchicine

Colchicine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the thing with colchicine and Panax ginseng: your body clears colchicine partly through a liver enzyme called CYP3A4, and ginseng can nudge that enzyme in either direction. The catch is that colchicine is an active drug (not a prodrug), so if ginseng slows that enzyme, colchicine can build up. That matters because colchicine has a narrow safety window, and higher levels can mean nausea, diarrhea, belly cramps, or in serious cases muscle and blood problems. If ginseng instead speeds clearance, colchicine could work less well for your gout.

Honestly, this interaction is mostly theoretical and the human evidence points in different directions, so I wouldn't panic. Still, since colchicine is one to respect, please run it by your pharmacist or doctor before combining them.

About Colchicine

What Is Colchicine Used For?

Brand names: Colchicine, Colsalide Improved, Mitigare, Colcrys, Gloperba, Lodoco

Colchicine tablets and capsules are used to prevent gout flares in adults, and colchicine tablets are also used to treat acute gout flares when taken at the first sign of a flare. Colchicine tablets are additionally approved for familial Mediterranean fever (FMF) — a hereditary inflammatory condition — in adults and children 4 years and older.

Lodoco (colchicine tablet) is specifically indicated to reduce the risk of heart attack, stroke, coronary revascularization, and cardiovascular death in adults with established atherosclerotic disease or multiple cardiovascular risk factors. Colchicine is not a pain reliever and should not be used for pain from other causes.
