Why this applies to Larotrectinib

Larotrectinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the situation with Larotrectinib (Vitrakvi) and Panax ginseng. Your cancer medication is processed by a liver enzyme called CYP3A4, and ginseng can nudge that enzyme in either direction. That makes its effect on Larotrectinib hard to predict. If ginseng slows the enzyme, more drug could build up, which may mean more side effects like dizziness, nausea, fatigue, or changes in liver tests. If ginseng speeds it up, your levels could drop and the medicine might not work as well against the tumor.

Honestly, this is mostly theoretical for Larotrectinib, but with a cancer therapy you don't want to gamble on either outcome. Please don't add ginseng without first checking with your oncology team or pharmacist.

About Larotrectinib

What Is Larotrectinib Used For?

Brand name: Vitrakvi

Vitrakvi is used to treat adult and pediatric patients with solid tumors that have a neurotrophic receptor tyrosine kinase (NTRK) gene fusion — a specific genetic alteration — without a known acquired resistance mutation. It is used when the tumor has spread (is metastatic) or when surgery would likely cause severe harm, and when there are no satisfactory alternative treatments or the cancer has continued to grow after prior treatment.

Patients are selected for Vitrakvi based on an FDA-approved test that confirms the NTRK gene fusion in tumor tissue. This approval was granted under the FDA's accelerated approval pathway, based on overall response rate and duration of response; continued approval may depend on results from confirmatory trials.
