Why this applies to Mavacamten
Mavacamten falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Panax Ginseng interacts with.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the thing with Camzyos (mavacamten): your liver clears it largely through an enzyme called CYP3A4, and Panax ginseng can nudge that enzyme in either direction. That's why I'd be cautious here. If ginseng slows the enzyme down, mavacamten can build up, and since it works by weakening the heart's squeeze, too much can drop your heart function too far, leaving you short of breath, dizzy, or swollen. If ginseng speeds the enzyme up instead, the drug may clear faster and not control your symptoms as well.
Mavacamten is carefully dose-adjusted with echocardiograms, so even modest swings matter. This is mostly theoretical, but please loop in your cardiologist or pharmacist before adding ginseng.

It blocks CYP3A4, the liver enzyme that clears mavacamten, and too much of this drug can weaken the heart's pumping strength, the very problem careful dosing exists to prevent.”

Camzyos (mavacamten) is approved for adults with symptomatic obstructive hypertrophic cardiomyopathy (HCM) — a condition where the heart muscle grows too thick and blocks blood flow out of the heart. Specifically, it is used in people whose symptoms fall into New York Heart Association (NYHA) class II or III, meaning they have noticeable limitations on physical activity.
The goal of treatment is to improve how much the patient can do physically and to reduce the symptoms that HCM causes, such as shortness of breath, chest pain, and fatigue.
