Why this applies to Nirmatrelvir, Ritonavir

Nirmatrelvir, Ritonavir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Panax Ginseng interacts with.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the short version: Paxlovid is built around how the body's CYP3A4 system handles medicines. In fact, the ritonavir piece is added on purpose to block CYP3A4 so the antiviral nirmatrelvir stays at a strong enough level to fight the virus. Panax ginseng can nudge that same enzyme in either direction, so the concern is mostly theoretical and the data are mixed.

The bigger picture is that ritonavir is a powerful enzyme blocker itself, so over a typical 5-day Paxlovid course, ginseng is unlikely to throw things off in a major way. Still, since you're only on this medicine briefly to clear an infection, it's reasonable to simply pause ginseng during treatment. Run it by your pharmacist or doctor first, since Paxlovid has many drug interactions of its own.

Although Panax ginseng has demonstrated variable effects on cytochrome P450 3A4 (CYP3A4), which metabolizes lopinavir, Panax ginseng is unlikely to alter levels of lopinavir/ritonavir.
Lopinavir is metabolized by CYP3A4 and is administered with the CYP3A4 inhibitor ritonavir to increase its plasma concentrations. Panax ginseng has shown variable effects on CYP3A4 activity in humans. However, taking Panax ginseng (Vitamer Laboratories) 500 mg twice daily for 14 days did not alter the pharmacokinetics of lopinavir/ritonavir in 12 healthy volunteers.

Here's the good news if you're taking Paxlovid (nirmatrelvir plus ritonavir) and wondering about ginseng. Paxlovid works because the ritonavir piece blocks a liver enzyme called CYP3A4, which lets the nirmatrelvir stay at strong, effective levels in your body. Panax ginseng has shown mixed, back-and-forth effects on that same enzyme in studies, so on paper you might worry it could nudge those levels around.

In practice, though, a real study in healthy volunteers taking 500 mg of ginseng twice daily for two weeks found no meaningful change in lopinavir/ritonavir levels, which is a close cousin of Paxlovid. So this interaction is rated minor and unlikely. Since Paxlovid is short-term (usually five days) and has many other interactions, it's still smart to run any supplement past your pharmacist first.
