Theoretically, Panax ginseng has an additive effect with drugs that prolong the QT interval and potentially increase the risk of ventricular arrhythmias. However, research is conflicting.
Clinical research shows that short-term use of Panax ginseng can increase the QT interval. However, no changes in QT interval have been identified with prolonged use.

Here's the thing to know about pairing Panax ginseng with pimozide. Pimozide already carries a well-known risk of affecting your heart's rhythm. It can stretch out something called the QT interval, which is part of your heartbeat's electrical cycle. When that gets too long, it can rarely trigger a dangerous irregular heartbeat. This is why pimozide tends to be used only when other medicines haven't worked.

Some short-term studies suggest ginseng may slightly lengthen the QT interval too, so stacking it on top of pimozide could, in theory, add to that risk. That said, the evidence is mixed and longer-term use didn't show this change. I wouldn't panic, but I would loop in your pharmacist or doctor before starting ginseng, since pimozide is a medicine where heart rhythm really matters.

Pimozide also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, Panax ginseng might increase or decrease levels of drugs metabolized by CYP3A4.
Panax ginseng may affect the clearance of drugs metabolized by CYP3A4. One such drug is imatinib. Inhibition of CYP3A4 was believed to be responsible for a case of imatinib-induced hepatotoxicity. In contrast, Panax ginseng has been shown to increase the clearance of midazolam, another drug metabolized by CYP3A4. Clinical research shows that Panax ginseng can reduce midazolam area under the curve by 44%, maximum plasma concentration by 26%, and time to reach maximum plasma concentration by 29%. Midazolam metabolism was also increased in animals given Panax ginseng. Until more is known, use Panax ginseng cautiously in combination with CYP3A4 substrates.

Here's the situation with pimozide (Orap) and Panax ginseng. Your liver uses an enzyme called CYP3A4 to clear pimozide from your body, and ginseng can nudge that enzyme in either direction. The catch is that pimozide is a drug we watch very closely, because higher blood levels can mess with your heart's rhythm (a problem called QT prolongation) and lead to dizziness, fainting, or dangerous heartbeats. If ginseng slowed your clearance, more pimozide could build up. If it sped things up, your tics might be less controlled.

Honestly, this is mostly theoretical and the data are mixed, but because pimozide has so little wiggle room for error, I'd play it safe and check with your pharmacist or doctor before adding ginseng.

What Is Pimozide Used For?

Brand name: Orap

Pimozide tablets are used to suppress the motor tics (involuntary movements) and phonic tics (involuntary sounds or vocalizations) that occur in Tourette's Disorder. It is reserved for patients whose daily life or development is severely affected by these tics.

Pimozide is not a first-choice treatment, and it is not meant for tics that are only mildly bothersome or cosmetic in nature. It should only be considered when standard treatments have not worked well enough.
