CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES

Thioridazine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Panax Ginseng interacts with.
There is some evidence that Panax ginseng can inhibit the CYP2D6 enzyme by approximately 6%. In addition, in animal research, Panax ginseng inhibits the metabolism of dextromethorphan, a drug metabolized by CYP2D6, by a small amount. However, contradictory research suggests Panax ginseng might not inhibit CYP2D6. Until more is known, use Panax ginseng cautiously in patients taking drugs metabolized by these enzymes.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the idea behind this one. Your body uses a liver enzyme called CYP2D6 to clear thioridazine. Panax ginseng might slow that enzyme down a little, which could let thioridazine build up more than usual. Thioridazine isn't a prodrug, so higher levels would mean stronger effects and a greater chance of side effects like drowsiness, dizziness, dry mouth, or a fast or irregular heartbeat.
This matters more with thioridazine than with most drugs, because it can affect heart rhythm (a change called QT prolongation), and even small increases can be a concern. That said, the evidence here is weak and conflicting, with one study showing only about a 6% effect. I wouldn't panic, but please run it by your pharmacist or doctor before combining them.

QT INTERVAL-PROLONGING DRUGS

Clinical research shows that short-term use of Panax ginseng can increase the QT interval. However, no changes in QT interval have been identified with prolonged use.

Here's the thing with thioridazine (Mellaril): it's well known for affecting the heart's rhythm by stretching out something called the QT interval. That's actually one of the big reasons it's reserved for people who haven't done well on other options. Panax ginseng may also nudge the QT interval a bit, at least with short-term use, so the worry is that the two could add up and raise the small risk of a dangerous irregular heartbeat.
I want to be straight with you though: this is mostly theoretical, and the research is mixed. Some studies show no QT change with longer ginseng use. Still, because thioridazine is already a heart-rhythm risk drug, I'd run the ginseng by your pharmacist or doctor before combining them rather than guessing.

Thioridazine Hydrochloride tablets are used for the management of schizophrenia — but only in patients who have already tried at least two other antipsychotic medications without adequate results, either because those drugs didn't work well enough or caused intolerable side effects.
Because thioridazine carries serious heart-rhythm risks, it is considered a last-resort option for schizophrenia, not a first-line treatment. It is not approved for dementia-related psychosis or any other psychiatric condition.
