Drospirenone, Estetrol falls under ESTROGENS — the group Progesterone interacts with.

Theoretically, using estrogen along with progesterone supplements or non-prescription bioidentical progesterone products might alter the effects of estrogen.
Concomitant use of estrogen with prescription progesterone products can cause breast tenderness. Also, use of conjugated equine estrogens with oral micronized progesterone in postmenopausal patients seems to blunt the beneficial effects of estrogen on the lipoprotein profile, although it might not affect estrogen-induced reduction in plasma lipoprotein (a). It is unclear if this interaction would be clinically significant with the doses found in oral or topical progesterone supplements or non-prescription bioidentical products.

Nextstellis already contains both an estrogen (estetrol) and a progestin (drospirenone), so it is a complete birth control pill. Adding a separate progesterone supplement or a 'natural' progesterone cream on top of it means you would be stacking extra hormone activity that your pill is already providing.

The main thing reported here is more breast tenderness, and possibly small shifts in how estrogen affects your cholesterol numbers. This is largely theoretical at supplement doses, and the over-the-counter creams are not well standardized, so we honestly cannot predict their effect well. It is unlikely to be dangerous, but since you do not need extra progesterone while on this pill, check with your pharmacist or doctor before combining them.
