Why this applies to nilotinib

nilotinib falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the gist with propolis and nilotinib (Tasigna). Your body relies on a liver enzyme called CYP3A4 to break down nilotinib. In the test tube, some propolis compounds appear to slow that enzyme down. If that actually happened in people, nilotinib could pile up in the blood and lead to more of its known issues, like nausea, fatigue, rash, headache, and most importantly heart rhythm changes (a longer QT interval), which is one reason this drug is monitored so carefully.

The reassuring part: this is theoretical. It comes from lab and animal studies, not from real patients, and the propolis pieces that block CYP3A4 don't seem to survive well once swallowed. Still, since nilotinib has a narrow safety margin, please run any propolis supplement by your oncologist or pharmacist before combining them.

Propolis is a natural, resin-like substance made by bees that has antimicrobial and anti-inflammatory properties in lab studies. Early research suggests it may help with cold sores, mouth sores, and minor wounds, but high-quality evidence is limited for most uses. What Is Nilotinib Used For?

Brand name: Tasigna

  • Nilotinib is used to treat certain types of chronic myeloid leukemia (CML; a type of cancer of the white blood cells) who have recently found to have this condition in adults and children 1 year of age and older. It is also used to treat certain types of CML in adults whose disease could not be treated successfully with imatinib (Gleevec) or adults who cannot take imatinib because of side effects. Nilotinib is also used to treat certain types of CML in children 1 year of age or older whose disease could not be treated successfully with other tyrosine kinase inhibitor therapies or who cannot take these medications because of side effects. Nilotinib is in a class of medications called kinase inhibitors. It works by blocking the action of the abnormal protein that signals cancer cells to multiply. This helps to stop or slow the spread of cancer cells.
