Why this applies to Tacrolimus

Tacrolimus falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Tacrolimus is one of those medications where the blood level really matters. Your transplant team checks it closely because too little can let your body reject the organ, and too much can cause real problems like kidney strain, tremors, headaches, high blood pressure, and higher blood sugar. Your liver clears tacrolimus using an enzyme called CYP3A4.

The concern here is mostly on paper. In lab dishes, propolis has sometimes slowed CYP3A4, which could in theory let tacrolimus build up. But other lab studies showed no effect, and in animals it did not change CYP3A4 in any meaningful way. This has never been reported in people. Still, because tacrolimus has such a narrow safety window, I'd loop in your transplant pharmacist before adding propolis.

Tacrolimus is prescribed to help prevent your body from rejecting a transplanted organ. It is used in adults who receive a kidney, liver, or heart transplant, and in children who receive a liver transplant. It is always used alongside other anti-rejection medicines.

Because tacrolimus works by dampening your immune system, it lowers the risk of your body attacking the new organ — but it also means you need to be watched closely for infections and other side effects throughout your treatment.
