CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Vorapaxar

Vorapaxar falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Propolis interacts with.

Theoretically, high doses of propolis might increase blood levels of drugs metabolized by CYP3A4.
Some in vitro research shows that propolis extract can inhibit CYP3A4; however, other in vitro research shows that propolis has no effect on CYP3A4 activity. Furthermore, animal research shows that propolis extract does not significantly affect CYP3A4 activity when administered to rats at doses up to 250 mg/kg. It is postulated that the constituents of propolis that might in inhibit CYP3A4 in vitro do not have significant effects in vivo due to low bioavailability and hepatic first-pass effect. This effect has not been reported in humans.

Here's the quick version: your body uses a liver enzyme called CYP3A4 to clear vorapaxar (Zontivity). In a test tube, some propolis compounds appear to slow that enzyme down, which in theory could let vorapaxar build up a bit. Since vorapaxar is a blood thinner that keeps platelets from clumping, higher levels could mean a greater chance of bruising or bleeding, such as nosebleeds, bleeding gums, or blood that's slow to stop.

The good news is that this is very theoretical. Lab and animal studies disagree, and it has never actually been reported in people. Small amounts in a lozenge probably aren't a concern, but concentrated supplement doses are less studied. Since vorapaxar already carries bleeding risk, just mention any propolis use to your pharmacist or doctor before starting.

Theoretically, propolis might increase the risk of bleeding when taken with antiplatelet or anticoagulant drugs.
In vitro research shows that propolis water extract and the propolis constituent, caffeic acid phenethyl ester, can inhibit platelet aggregation. Additionally, evidence from an animal model shows that taking propolis in addition to warfarin decreases INR, suggesting that propolis can decrease the effectiveness of warfarin.

Here's the thing to keep in mind: Vorapaxar already thins your platelets to prevent clots, and you're usually taking it alongside aspirin and clopidogrel. That's a fair amount of bleeding protection working at once. Propolis may add a little to that effect, because in lab studies its compounds (like CAPE) can also make platelets less sticky.

So in theory, stacking propolis on top of Vorapaxar could nudge your bleeding risk a bit higher, think easier bruising, longer bleeding from a small cut, nosebleeds, or bleeding gums. I'll be honest though: this is based only on test-tube and animal research, so it's a theoretical concern, not a proven one. Still, with a blood thinner I'd run propolis past your pharmacist or doctor first.

Propolis is a natural, resin-like substance made by bees that has antimicrobial and anti-inflammatory properties in lab studies. It is generally well tolerated, but allergic reactions can occur, especially in people allergic to bee products.

Zontivity (vorapaxar) is used to lower the risk of heart attack, stroke, cardiovascular death, and the need for urgent procedures to open blocked arteries. It is prescribed for people who have already had a heart attack or who have peripheral arterial disease (PAD) — a condition where arteries outside the heart become narrowed or blocked.

Zontivity is not used alone. It is given alongside aspirin and/or clopidogrel as part of a broader antiplatelet treatment plan.
