Slippery Elm and Acalabrutinib

Here is the interaction we found between these two, with the evidence behind it.

Why this applies to Acalabrutinib

Acalabrutinib falls under ORAL DRUGS — the group Slippery Elm interacts with.

Theoretically, slippery elm may slow the absorption and reduce serum levels of oral drugs.
Slippery elm inner bark contains mucilage, which may interfere with the absorption of orally administered drugs.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist with Calquence (acalabrutinib) and slippery elm. That slick, gel-like fiber (mucilage) in slippery elm could, in theory, coat your gut and grab onto a pill as it passes through. The worry is that it slows or blocks how much acalabrutinib your body actually absorbs, leaving less of the cancer-fighting medicine in your bloodstream. With a serious drug like this, you don't want a weaker dose.

I'll be honest: this is a theoretical concern based on how the herb behaves, not something proven in studies. Still, since acalabrutinib treats lymphoma and leukemia, I'd play it safe. A simple fix is timing. Don't stop or change anything on your own. Please run it by your oncology team or pharmacist first, ideally spacing the two apart by a few hours.

About Slippery Elm

What is Slippery Elm?

Slippery Elm
  • Sore throat and cough
  • Heartburn and acid reflux
  • Digestive upset and IBS
  • Skin irritation (topical)
  • Constipation or diarrhea

Slippery elm is a traditional herbal remedy made from the inner bark of a North American elm tree, used mainly to soothe sore throats and irritated digestive tracts. Its mucilage can coat and calm mucous membranes, but solid human evidence for most uses is limited. It is generally considered low-risk for short-term use, but you should still check with your pharmacist or doctor first.

About Acalabrutinib

What Is Acalabrutinib Used For?

Brand name: Calquence

Calquence is used to treat adults with mantle cell lymphoma (MCL) — either newly diagnosed (in combination with bendamustine and rituximab, for patients who are not candidates for a stem cell transplant) or previously treated (after at least one prior therapy). It is also used to treat adults with chronic lymphocytic leukemia (CLL) or small lymphocytic lymphoma (SLL).

Both the capsule and tablet forms of Calquence are approved for all three of these uses. The specific combination regimen and treatment schedule depend on the condition being treated and what other medicines are used alongside it — your care team will map that out for you.
