Theoretically, concomitant use of mangosteen with anticoagulant or antiplatelet drugs may increase the risk of bleeding.
In vitro and animal research shows that gamma-mangostin, a constituent of mangosteen, is a potent and competitive antagonist of the serotonin 2A (5-HT2A) receptor. Antagonism of the 5-HT2A receptor is believed to reduce platelet aggregation.

Theoretically, concomitant use of mangosteen with donepezil might increase the effects of donepezil.

Overview

Mangosteen (Garcinia mangostana) is a tropical fruit native to Southeast Asia. It has a thick, deep purple rind and soft, sweet white segments inside. The fruit is enjoyed fresh in many parts of the world, but in supplements it is the rind, or pericarp, that gets the most attention.

The rind contains plant compounds called xanthones, which are antioxidants. Because of these compounds, mangosteen has been marketed in juices, capsules, and powders for a wide range of health claims. It has also been used in traditional medicine in some regions.

Today you will most often see mangosteen sold as a juice blend or as a supplement promising antioxidant, anti-inflammatory, or immune benefits.

How it works

The main active compounds in mangosteen are xanthones, especially one called alpha-mangostin. These are found mostly in the fruit's rind. In laboratory studies, xanthones act as antioxidants, meaning they may help neutralize unstable molecules called free radicals.

Lab and animal research also suggests xanthones may lower certain inflammation signals and have antimicrobial activity. Some studies hint at effects on blood sugar or cell growth.

It is important to remember that most of this work comes from cells in dishes or from animals. Whether these effects happen in the human body, at the amounts found in supplements, is not well established.

Safety & precautions

Mangosteen eaten as a fruit is generally considered safe for most people. The safety of concentrated supplements and high-dose extracts taken over a long time is less clear because they have not been well studied.

There have been reports linking heavy use of mangosteen juice to a buildup of acid in the blood (lactic acidosis) in some people, though this is rare. People with kidney problems, bleeding disorders, or who are scheduled for surgery should be especially cautious and check with a healthcare provider first.

Pregnancy and breastfeeding: There is not enough reliable information about using mangosteen supplements during pregnancy or breastfeeding. To be safe, avoid supplement amounts during these times and stick to normal food amounts only if your doctor agrees.

Always tell your pharmacist or doctor about any supplement you take, especially if you have a health condition or take medications.

Side effects

Mangosteen is usually well tolerated as a food. When side effects occur, they are typically mild and may include stomach upset, diarrhea, or constipation.

Myasthenia gravis Renal

Orally, mangosteen extract up to 560 mg daily has been reported to cause abnormal urination. It is not known if this side effect was related to mangosteen. In one case report, a patient with chronic kidney disease and metabolic syndrome consumed mangosteen juice daily for 12 months and later presented with severe lactic acidosis. The juice was reported to contain 250 mg of mangosteen with 25 mg alpha-mangostin per ounce. Alpha-mangostin appears to inhibit mitochondrial function, disrupting the electron transport chain and adenosine triphosphate (ATP) production, causing accumulation of reactive oxygen species and inducing apoptosis. Researchers speculate that these effects might have led to lactic acidosis in this patient.
