Iloprost falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Mangosteen interacts with.

Theoretically, concomitant use of mangosteen with anticoagulant or antiplatelet drugs may increase the risk of bleeding.
In vitro and animal research shows that gamma-mangostin, a constituent of mangosteen, is a potent and competitive antagonist of the serotonin 2A (5-HT2A) receptor. Antagonism of the 5-HT2A receptor is believed to reduce platelet aggregation.

Here's the thing to know about iloprost (Ventavis): besides relaxing the blood vessels in your lungs, it also makes your platelets a little less sticky, which can slightly raise bleeding risk on its own. Mangosteen may do something similar. Lab and animal studies suggest one of its compounds can block a receptor involved in platelet clumping, so pairing the two could in theory make bleeding easier, think more bruising, nosebleeds, or bleeding gums.

I want to be honest that this is theoretical and only based on test-tube and animal research, not human studies. Food amounts of the fruit are unlikely to matter. Concentrated supplements or juices are where I'd be more cautious. Before adding a mangosteen supplement, check with your pharmacist or doctor.
