Theoretically, concomitant use of mangosteen with donepezil might increase the effects of donepezil.
Animal research shows that concomitant use of an aqueous extract of mangosteen pericarp with donepezil increases brain concentrations of donepezil at 4 hours by 64% without associated effects on systemic exposure.

Namzaric combines two Alzheimer's medicines: memantine and donepezil. The piece that matters here is the donepezil. We have a small bit of animal research hinting that mangosteen might nudge more donepezil into the brain (one study saw about a 64% bump in brain levels at 4 hours), even though the amount in the bloodstream didn't really change.

If that held true in people, the worry would be a little more donepezil effect, which could mean more nausea, diarrhea, loss of appetite, vivid dreams, or a slower heartbeat. But honestly, this is theoretical and based only on lab and animal work, so I wouldn't lose sleep over it. Eating mangosteen fruit is unlikely to matter; concentrated supplements are the bigger question. Before adding any mangosteen supplement, just run it by your pharmacist or doctor.
