Cilostazol falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Mangosteen interacts with.

Theoretically, concomitant use of mangosteen with anticoagulant or antiplatelet drugs may increase the risk of bleeding.
In vitro and animal research shows that gamma-mangostin, a constituent of mangosteen, is a potent and competitive antagonist of the serotonin 2A (5-HT2A) receptor. Antagonism of the 5-HT2A receptor is believed to reduce platelet aggregation.

Here's the gist: cilostazol already works to keep your platelets from clumping together, which is how it improves blood flow in your legs. Mangosteen may have a mild, similar effect of its own. In lab and animal studies, one of its compounds (gamma-mangostin) appears to block a receptor involved in platelet clumping. Stack that on top of cilostazol and, in theory, you could nudge your bleeding risk a little higher.

I want to be honest about the strength here: this is based on test-tube and animal work, not real people, so it's more of a caution than a proven problem. Still, watch for easy bruising, nosebleeds, or bleeding that's slow to stop. Enjoying the fruit is unlikely to matter much, but before starting concentrated mangosteen supplements, please run it by your pharmacist or doctor.
