Antithrombin III falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Mangosteen interacts with.

Theoretically, concomitant use of mangosteen with anticoagulant or antiplatelet drugs may increase the risk of bleeding.
In vitro and animal research shows that gamma-mangostin, a constituent of mangosteen, is a potent and competitive antagonist of the serotonin 2A (5-HT2A) receptor. Antagonism of the 5-HT2A receptor is believed to reduce platelet aggregation.

Here's the short version: Antithrombin III (Thrombate III) is a blood thinner given by IV, usually to prevent clots in people who are low on this natural protein. Mangosteen doesn't change how this medicine is processed in your body. Instead, lab and animal studies hint that a compound in mangosteen (gamma-mangostin) may make platelets a little less sticky, which could add to the clot-slowing effect you already get from Antithrombin III.

Put simply, stacking the two might nudge your bleeding risk up a bit, things like easier bruising, nosebleeds, or bleeding that's slow to stop. Keep in mind this is theoretical and based only on lab work, not on people. Eating the fruit is unlikely to matter, but before using concentrated mangosteen supplements, please check with your doctor or pharmacist.
