Defibrotide Sodium falls under ANTICOAGULANT/ANTIPLATELET DRUGS — the group Mangosteen interacts with.

Theoretically, concomitant use of mangosteen with anticoagulant or antiplatelet drugs may increase the risk of bleeding.
In vitro and animal research shows that gamma-mangostin, a constituent of mangosteen, is a potent and competitive antagonist of the serotonin 2A (5-HT2A) receptor. Antagonism of the 5-HT2A receptor is believed to reduce platelet aggregation.

Here's the basic idea: defibrotide is a clot-fighting medicine given in the hospital to keep your blood from forming clots while your liver heals after a stem cell transplant. Mangosteen contains a compound (gamma-mangostin) that, in lab and animal studies, seems to make platelets less sticky. Stack those two together and the concern is a little extra bleeding risk, things like easy bruising, nosebleeds, bleeding gums, or harder-to-stop bleeding.

I'll be honest, this is a theoretical worry based only on test-tube and animal research, not strong human data, so it may not amount to much. Still, since defibrotide is used in a serious hospital setting, please tell your transplant team or pharmacist about any mangosteen juice or supplements rather than adding them on your own.

What Is Defibrotide Injection Used For?

Brand name: Defitelio

Defitelio (defibrotide injection) is used to treat hepatic veno-occlusive disease (VOD) — also called sinusoidal obstruction syndrome (SOS) — in adult and pediatric patients. This is a serious condition where small blood vessels in the liver become blocked, most often after a hematopoietic stem-cell transplant (HSCT).

Defitelio is specifically indicated when VOD is accompanied by kidney dysfunction or pulmonary (lung) dysfunction, making it a treatment for one of the most severe forms of this complication.
