Theoretically, concomitant use of mangosteen with donepezil might increase the effects of donepezil.
Animal research shows that concomitant use of an aqueous extract of mangosteen pericarp with donepezil increases brain concentrations of donepezil at 4 hours by 64% without associated effects on systemic exposure.

Here's the gist with your donepezil patch (Adlarity) and mangosteen: some early animal work hints that mangosteen might nudge more donepezil into the brain, without changing how much is in the bloodstream. In theory, that could slightly amplify donepezil's effects, meaning you might notice more of its usual side effects like nausea, diarrhea, loss of appetite, muscle cramps, a slower heart rate, or trouble sleeping.

I want to be honest, though: this is a minor, theoretical concern based only on a lab-style animal study, not real people. Eating mangosteen as a fruit is very unlikely to matter. Concentrated supplements or juices are where I'd be a bit more cautious. Before adding a mangosteen supplement, just run it by your pharmacist or doctor so you can watch for any changes together.
