Donepezil (patch) falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Sage interacts with.

Theoretically, sage might increase the levels and clinical effects of drugs metabolized by CYP2D6.
In vitro evidence suggests that aqueous extracts of sage can inhibit CYP2D6. So far, this interaction has not been reported in humans.

Here's the gist for your donepezil patch (Adlarity): your body partly relies on a liver enzyme called CYP2D6 to break donepezil down. Sage, at least in lab dish studies, seems to slow that enzyme a bit. If it did the same thing in real life, donepezil could build up slightly and its effects, both good and unwanted, could get stronger.

With donepezil, that usually means more of the classic side effects: nausea, diarrhea, loss of appetite, dizziness, a slower heartbeat, or trouble sleeping. Now the reassuring part. This is theoretical, based only on test-tube work, and it has never actually been reported in people. A little sage in cooking is very unlikely to matter. Concentrated sage extracts or capsules are the bigger question mark, so run those by your pharmacist or doctor first.

In vitro evidence suggests that common sage (Salvia officinalis) and Spanish sage (Salvia lavandulaefolia) can inhibit acetylcholinesterase and might increase acetylcholine levels.

Here's the idea behind this pairing. Your donepezil patch (Adlarity) works by blocking an enzyme called acetylcholinesterase, which raises the amount of acetylcholine in your brain and helps with memory symptoms. Interestingly, lab studies suggest sage may block that very same enzyme. So in theory, sage could add to what donepezil is already doing and gently push its effects a little higher.

If that happened, you might notice more of donepezil's usual side effects, like nausea, an upset stomach, diarrhea, more sweating, a slower heartbeat, or muscle cramps. I want to be honest though: this concern comes only from test-tube research, not from people, so it's far from proven. Cooking with sage is very unlikely to matter. Before using concentrated sage extracts or sage oil, just check with your pharmacist or doctor.

Sage is a common kitchen herb that is generally safe in food amounts and is traditionally used for sore throats, digestion, sweating, and memory. Some early research is encouraging for sore throat and menopausal hot flashes, but most uses are not well proven. Concentrated extracts and the essential oil should be used carefully because high doses contain a compound called thujone, which can be harmful.
